All Members:
Please complete the survey below in order to update our records and to enable the Elmira Golf Club to contact our Members more effectively regarding future Events, Changes, Announcements, Annual Meetings, Special Meetings, etc.

1. Complete and return the form below by completing form and hitting send.
2. Print a copy of the form by Clicking the Print Button on the bottom of the form and completing the form and sending or dropping it off at the Elmira Golf Club.
